The United States Chemical Safety Board (CSB) is an independent federal agency charged with investigating industrial chemical accidents.  In the wake of several high profile academic safety incidents (UCLA, Texas Tech) the CSB performed its first-ever investigation of lab safety.  Their video findings are here; it is 24 minutes well spent.

The incidents at Yale and U Hawaii happened after this video, so clearly not all lessons learned have been applied it would seem.
